NCL CYP Complex Discharge Team Administrator

Receive and process all referrals triggered by the clinical staff. To record and monitor the above information on systems/excel sheets so that the referral trail is auditable. The information is also used to produce trends, analysis, and reports at request. Receive and follow up on outstanding information through liaising with the wards and other Complex Discharge colleagues.
These updates should be discussed with the Complex Discharge Nurses and added to the relevant systems in a clear and concise manner as the information forms part of a clinical record and is also referred to when asked to provide analysis/ reports. Provide intelligence on Complex Discharge performance. Work will involve interrogating, collecting/inputting, and maintaining various databases to produce spreadsheet reports for the Complex Discharge team. Develop knowledge of the Discharge Management System and use within the everyday role to support discharge planning for patients who have care needs following their acute stay Support the daily activities around patient choice as directed by the Complex Discharge Specialist Nurses.
Required to provide reports or take minutes as required. Provide an administrative and secretarial service to the Complex Discharge Team including maintaining a diary for the Complex Discharge Team. Required to order stationery/ equipment required by the team using the appropriate system, to raise requisitions and receipt invoices where applicable. Accurate record keeping is essential.
To receive all incoming calls, prioritising them accordingly and logging patient referrals when appropriate. Participate in the receiving and distribution of all internal and external mail, And logging and/or redirecting as appropriate. To maintain and keep up to date an accurate filing system, both paper and electronic, highlighting any anomalies or improvements to the clinical manager. To liaise with outside agencies and voluntary organisations on behalf of the Complex Discharge Team, e.g., parents, carers, young people, residential placement Homes, Social Work Area Offices, GPs, Clinical Commissioning Groups, Mental Health Teams, Courts, Police, Patients and Relatives.
To liaise with internal staff across NCL paediatric ward areas that includes all disciplines. In conjunction with the Team Leader to participate in annual appraisal, identify own self-development and training needs. Where required, to visit the wards to support discharge planning.
