Health Inequalities Lead
at East Suffolk and North Essex NHS Foundation Trust Other in Other (Published at 21-02-2025)
To strategically lead the implementation and delivery of the ESNEFT Inequalities strategy, working collaboratively with partners across the Integrated Care System (ICS) and putting our patients at the heart our approach.
Ensuring equitable access to our services and improve health outcomes for all our patients, supported by these key objectives;
Get everyone involved in Equity
Identify and monitor health and healthcare inequalities using data
Understand the causes of inequities and barriers resulting in them
Create change together with our partners and communities to measure its impact
ESNEFT Employees will be offered on a secondment basis.
Ensure that the Inequalities Strategy and Programme is planned and led effectively, ensuring safe and seamless integration with other existing processes and services
.
Take the lead in building capability across the Trust in Quality Improvement methods including Making Every Contact Count (MECC). Support frontline staff to test, implement and spread sustainable improvements to improve equity of care across the Trust and the wider health and social care system.
To lead specific Trust-wide inequalities projects, ensuring outcomes support equitable health promotion and the prevention agenda
