Are you looking for a new challenge where you can make a difference in the lives of vulnerable children and young people requiring support through the difficult period of transitioning out of Children’s Emotional Wellbeing Mental Health (EWMH) Services and into appropriate mental health or community services as they reach 18 years of age?

We have an exciting opportunity for a Support Worker to join Surrey & Borders NHS Foundation Trust (SABP) in the Reaching Out service. The Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network approach. You will be a part of the Transition Pathway, which provides support for 17 and 18-year-old’s as they transition out of Children’s EWMH Services.

We are looking for a clinician who is creative, highly motivated, shows initiative and is passionate and committed for ensuring high quality services for people with mental health needs.

There is 1 post covering the East of Surrey

You will case manage and co-ordinate support for young people aged 17-18 years old as they transition, developing a Transition Care Plan with the young person to identify their needs and ensure they offer the right help and support throughout their transition period and beyond.

You will accompany the young person to appointments, assessments and therapy, educate and reflect on the differences in culture and practice between services, as well as help to signpost and/or refer to services which will be emotionally beneficial for the young person.

You will be required to attend regular multi-disciplinary team meetings and liaise with a range of professionals, agencies and other parties involved in the young person’s care.

A key part of this role is building relationships and trust with young people, being comfortable having conversations and persevering with a young person about their needs, worries and resistance to engage.

This role is suitable for someone who has excellent knowledge of the mental health and community services that are available for young people across Surrey for young people.
