Homeless Mental Health Peer Support Worker Full Time

This is a very exciting opportunity to join a newly formed small team working with people experiencing homelessness with complex needs.

The team is currently made up of Nurses and Occupational Therapists, we work in partnership with CGL, P3 and Adult Social Care. We are looking to recruit a Psychologist, Psychology assistant and a Peer Support Worker to look at creative solutions to support people.

We are looking for a Peer Support Worker with lived experience that can work with individuals, the team and our partner organisations. We'd like you to bring your unique experience to the role to help us develop the team.

To work as part of a team supporting people with multiple complex mental health needs who are also experiencing homelessness.

Reporting directly to the team manager and under the professional supervision of the Peer Support Worker Lead.

To work across systems within GHC and across organisation, including Housing, Adult Social Care (ASC), Change Grow Live (CGL) substance misuse service, and other relevant partners.

To work in unconventional clinical environments (e.g. rough sleeper sites, supported housing, partner agency units, in the community etc) to deliver support to individuals with complex emotional needs.

To work with direct partners, team, senior management, commissioners, and partners to implement NICE guidelines NG214 ‘Integrated Health and Social Care for people experiencing homelessness’ within the Trust, the team and wider services.

The role of peer support worker has been developed specifically for people who have lived experience of complex needs and homelessness.

The successful candidate would be a highly valued member of the multi-disciplinary team, the PSW during the development phase will offer limited support and practical assistance to service users in order for them to regain control over their lives and their own unique recovery process.
