The post holder will work with people with varying cognitive deficits in a range of mental health and community settings and in client’s own homes.
Under the supervision of Occupational Therapists, Nurses, and other clinical staff, they will work as part of a multidisciplinary team, providing support and assistance in all areas required to meet service need.
The post holder will work closely with clients and their families delivering planned interventions, as advised by qualified team members, enhance the client experience. Working both independently and alongside peers, they will assist the assessment and support of clients experiencing difficulties related to their mental health, helping to develop and implement programmes of care.

Work closely with team members and peers, to assist in delivering appropriate care.
Work with OT colleagues, providing practical support to help clients achieve their occupational goals.
Work with Non Medical Prescribers, making medication call checks under the supervision of a named professional.
Implement agreed programmes of care with clients and carers under the supervision of a named professional.
Use the Trust electronic systems to provide relevant information and support to clients.
Observe confidentiality regarding clients at all times .
Document clients’ presentation, progress and any significant events in clinical notes and ensure that these entries are counter-signed by a registered clinician.
Contribute to ongoing risk assessments by identifying signs of deterioration, monitoring clients’ progress, level of functioning and mental state and communicating outcomes and conclusions with staff involved in their care.
Report untoward occurrences, at the earliest opportunity, to a named, qualified key worker

Perform duties under the direction and indirect supervision of a named qualified professional.
Recognise when to seek support or escalate a client’s needs or risks to qualified team members.
Undertake ECG, Pulse, Blood Pressure, weight with patient either in patients own home or community/hub setting
Take information by telephone and provide telephone support to clients.
Maintain positive relationships within the multidisciplinary team and all others involved in the care of the client.
Maintain appropriate records regarding clients’ care, observations made and information provided.
Arrange/attend discussions regarding client care as and when required.
Build therapeutic relationships with the clients and carers and provide reassurance and information regarding the care pathway.
Provide input into the evaluation of the clients care pathway.
To encourage clients to be fully involved and engaged with their agreed care pathway and to access appropriate services.
Promote equality for all individuals by promoting and supporting individuals’ rights and choice.
Take a role in health promotion both with the clients, carers and other staff.
To assist in keeping the care plan/pathway up to date making accurate entries, regarding the clients’ wellbeing, as soon as practicably possible.
To work flexibly to service requirements within contracted hours.
To organise time effectively.
To arrange/attend meetings to discuss client care when required.
To utilise the Trusts computer system appropriately.
To organise referral on to other services, as required.
To be able to work alone in clients’ homes, or community settings, using own initiative as required.
To be aware of risks associated with being a lone worker in a community setting, following the lone worker policy.
