Discharge Co-ordinator Full Time

An exciting opportunity has arisen for a senior Registered Nurse or Therapist to join our team of Discharge Coordinators

The primary function of this role will be to work within the integrated discharge team identifying pathways for medically optimised patients.

This role involves working closely alongside, nursing, therapies, medical staffing and local authority as a decision-maker and facilitator of safe discharge from acute care. The role will also include community based work to support patients newly discharged from acute care or aid in prevention of hospital admission.

The post holder will act as a facilitator and decision-maker, coordinating activity across a range of organisations. Practitioners will assist with the facilitation of early safe discharge for hospital inpatients.

The post holders’ main function will be to act as a clinical decision-maker providing the point of contact and communication link between the hospital, locality team and primary care in order for clinical decision-making to take place.

The role will also require community working and managing the short term care team to facilitate patients step down from acute that require rehabilitation within the community and their own homes you will also be required to be the case holder for these patients and to support hospital avoidance for these patients

The role will primarily center within the Integrated Discharge team, however, at times support all elements of patient care as below alongside experienced colleagues specialised in these roles:

Patients at high risk of unplanned hospital admission (including those in care homes)

Patients requiring discharge from hospital (planned and unplanned) including support with fast track and facilitation of end of life discharges

Patients with complex health and social care packages following this period, admissions, assessments for a defined group of patients and subsequent signposting/discharge to other professionals as required.
