Activities Coordinator

To liaise with the nursing and therapy staff regarding the clients abilities, needs and desires Risk assess and evaluate the activities offered, seek the views of the clients and review general and individual programmes as required Identify the needs of the individuals and collective group and tailor the leisure and recreational activities and life skill programme to suit them ensuring each persons information is included in their personal support plans and on individual timetables To develop a 7-day activity schedule to reflect the needs and interests of the current client group To work with the care and rehab support workers to increase engagement in supporting client activities Actively participate in the activities, both at CRC and in the community, offering support and guidance to the clients and staff members as required Research and seeking out relevant opportunities Coordinate meetings with external providers to arrange bookings and discuss requirements and costings Planning and coordinating the wider project activities Liaison with fundraising Gathering feedback and liaising with clients, families and staff Administrative duties, phone messages, timetabling and diary updates Complete thorough risk assessments associated with the activities and take responsibility for the health, safety and welfare of the clients Working as part of the multidisciplinary team, liaise with external agencies and maintaining accurate, concise, up to date and timely records and reports as required by the service in electronic and hard copy, whilst maintaining confidentiality as appropriate Undertake any personal care tasks as required when working with a client at CRC, or the community ensuring dignity is always maintained Ensure that all relevant information relating to the clients (e.g. concerning behaviour, personal development, medical incidents etc.) is relayed to appropriate staff members and recorded in accordance with QEF policy and procedures Participate in staff meetings and reviews of the service, and contribute to the continuous evaluation and improvement of the service To identify specific training needs for the staff team in relation to the leisure and activity programme To assist in the organisation and implementation of individual, group and fundraising activities to promote and support each clients aspirations
